Our Adult Social Care department are looking for an Advanced Practitioner to join their Community Team 2 to provide professional support, leadership and line management to qualified and some unqualified social workers and other staff.
You will be part of an integrated multi-disciplinary team to help make a difference to lives.
This role will need you to be able to provide leadership on risk and safeguarding social worker matters and to promote personalised care pathways ensuring choice and control for all eligible individuals. This is a highly rewarding role where you will be working with others to influence and shape the local environment to promote social inclusions and accessibility.

Our ideal candidate will be an innovative high achiever who is able to demonstrate a high level of competence in Social Work practice. You’ll need to be able to communicate effectively across a diverse customer base.
You will have the opportunity to develop on your already existing in depth knowledge and understanding of Social Care legislation and regulations. This would include personalisation, eligibility criteria, rehabilitation and reablement, Safeguarding and MCA, DOLS, Multi-disciplinary complex case work, CQC Care Standards and Telecare.
We do ask that you have a degree or equivalent in Social Work and registered with HCPC.
This is a public facing role so you’ll need to show off your fluency in speaking and writing English.
